Required Documents and Appointment Scheduling

Preparing for your visit to a Kansas driver’s license office? Please select your service needed below to review the required documents and schedule your appointment.

If you have obtained a credential from another state, even if you were previously licensed in the state of Kansas, you will be required to present the documents listed below.

Please note:

  • If your out of state license is expired, you will be required to complete a vision, written, and driving exam.
  • If you have a hold on your license in any other states, you will not be eligible for a Kansas license or permit until that is resolved. You may obtain a Kansas identification card.
  • If you have an out of state permit, you will be required to complete testing in Kansas for a Kansas license or permit.
  • Please tape the names of those in the household to the inside of your mailbox, or notify your USPS driver of all residents, to prevent issues with non-delivery of your credential.

Required Documents

You will be required to provide at least one document from each section below.

1.) You will need to provide one lawful presence document listed below.

  • State Issued Birth Certificate
  • Unexpired Passport
  • Unexpired Permanent Resident Card
  • Unexpired Employment Authorization Card
  • Certificate of Citizenship or Certificate of Naturalization
  • Consular Report of Birth Abroad (FS-240)
  • If you are a foreign student, you will also need to present your I-20 or DS-2019

2.) You must provide proof of your Social Security Number.
Please select ONE document from the list below:

  • Social Security Card
  • Current W-2 or 1099 showing full Social Security Number
  • Current pay stub showing full Social Security Number

If you are a foreign student or dependent in the U.S. on a non-work authorized status, you will not need to provide SSN verification if you have not been issued a social security number.

3.) You must provide two recent proofs of current Kansas residential address. Proof must display your name, and show physical address, not a separate mailing or PO box only.

  • Utility Bill
  • Bank statement or letter from financial institution
  • Cancelled mail
  • Lease agreement
  • Title documents
  • Insurance documents

4.) If your current legal name does not match your lawful presence document, you will need to provide your legal name change document.

  • Certified State Issued Marriage Certificate
  • Court Ordered Divorce Decree
  • Any Court Ordered or Legal Name Change Documents
  • Legal Adoption Paperwork

5.) If you have an out of state credential, you must present one of the following from that state of issuance.

  • License or ID card
  • Motor Vehicle Record
  • Clearance Letter

6.) If you have completed driver’s education, you will want to present one of the following documents, as that may eliminate the need for testing in the office.

  • Certificate of Completion (Driver’s education permit, if only obtaining permit)
  • Letter from driver’s education stating Certificate of Completion was lost
  • If driver’s education was completed outside of Kansas, you must also present a letter from that state’s Department of Education stating that course is accredited.

7.) If you are a military veteran and would like the word “Veteran” printed on your credential, please present one document below.

  • DD-214 stating Honorable Discharge or Under Honorable Conditions
  • NGB-22 that states Honorable Discharge or Under Honorable Conditions
  • VA Card
  • Retired Military ID

To replace your Kansas credential, you will need to provide the following documents.

1.) Two proofs of identity. Select two from the list below.

  • State-Issued Birth Certificate
  • Student or Employee Identification Card
  • Expired License or Identification Card
  • Medical or Immunization Records
  • Unexpired Passport
  • Department of Corrections identification
  • Voter Registration Card
  • Professional License
  • W-2, Check Stub, or Tax Return
  • Vehicle Registration or Title
  • Disabled Identification Card
  • Military ID, DD-214, or NGB22
  • Certificate of Citizenship or Naturalization
  • U.S. Consular Report of Birth Abroad

If you are a non-U.S. Citizen, you will need to present your lawful presence document.

  • Valid Permanent Resident Card
  • Valid Employment Authorization Card
  • I-94
  • Foreign students will also need to present I-20 or DS-2019

3.) If you are a military veteran and would like the word “Veteran” printed on your credential, please present one document below.

  • DD-214 stating Honorable Discharge or Under Honorable Conditions
  • NGB-22 that states Honorable Discharge or Under Honorable Conditions
  • VA Card
  • Retired Military ID

Once your Kansas credential has expired over 1 year, you are considered a “new” application, and you will be required to provide at least one document from each section below. If your license expired over 1 year, you will be required to complete a vision, written, and driving exam.

To change the name on your Kansas credential, you will need to provide the following documents.

1.) Your Kansas license, permit, or identification card

2.) A piece of mail with your name and Kansas residential address. Mail may be in your previous name.

3.) Your legal name change document

  • Government issued marriage certificate
    • Cannot be “duplicate” marriage licenses or ceremonial church certificates
  • Divorce decree with name restored as declaration
  • Court order of Adoption
  • Court Order of Legal Name Change

4.) If you are a military veteran and would like the word “Veteran” printed on your credential, please present one document below.

  • DD-214 stating Honorable Discharge or Under Honorable Conditions
  • NGB-22 that states Honorable Discharge or Under Honorable Conditions
  • VA Card
  • Retired Military ID

To renew your credential, you will need to provide the following documents.

1.) Kansas license, permit, or identification card

  • Please note, permit renewals do require the written examination(s).

2.) A piece of mail with your name and current residential address.

If you have a commercial driver’s license, you will need to provide the following documents.

3.) DOT medical card, if you have one
